We kindly ask that you
respect all of our spa policies. We appreciate your business,
and want to make your stay here with us unforgettable. |

We
request the courtesy of a 24 hour cancellation or rescheduling
notice. We regret that cancellations of less than 24 hours will
be charged a 50% fee of the services booked. No Shows will be
charged a 100% fee of the services booked and Gift Certificates
will be null and void. NO EXCEPTIONS. When scheduling an appointment at Greenleaf Day Spa, you are agreeing to our cancellation policies and to the charge on your credit card or gift certificate when policies are violated.

| Greenleaf
Day Spa is not responsible for lost Gift Certificates. Gift Certificates
must be presented at the time of service. If not, you will be
charged for the service. Gift Certificates are non-refundable
and non- transferable. Unless noted, they have a one-year expiration.
Greenleaf Day Spa is not responsible for expired gift certificates
and we encourage our clients to use them as soon as possible.

We
regret that a late arrival will not receive an extension of scheduled
service times, and will be responsible for the full service fee.
We request you arrive 15 minutes before your appointment time.

Spa
services are available on a daily basis. However, we recommend
calling as far in advance as possible to ensure that your preferred
appointment times are available to you. A credit card or Gift
Certificate guarantee
is required for reservation of all spa services.

We
ask that all pagers and cell phones are turned off during your
time here with us to ensure complete relaxation for you, as well
as other guests visiting here with us.

We
suggest that you arrange care for your valubles prior to your
visit with us, as they will need to be removed before spa services.
Storage for your clothing and belongings is available. A spa robe
and slippers will be provided with package services. Greenleaf
Day Spa, Inc. is not responsible for any lost or damaged items.

Two
or more spa services are considered to be a "Spa Package."
An 18% gratuity will be added to any spa
package as is customary in the service industry. We work very
hard to ensure your visit with us is everything you expected and
more, and we appreciate your business.
